Your primary job will be to serve as the direct point of contact with all our elder law clients,  working closely with other key staff for the facilitation of all aspects of long-term care planning, case management, and Medicaid planning.

Essential Duties and Responsibilities* 

  • Facilitating and assuring the quality of all aspects of elder law services, working with life care plans to edit and modify based on the client’s individual situations, facilitating follow up appointments, completing forms as assigned, and implementing the plan as approved by the overseeing attorney. 

  • Provide comprehensive support to the supervising attorney by handling administrative tasks and managing client cases and needs. 

  • Guide families on advocacy on behalf of the client to facilities, staff, and medical team.  

  • Assess clients for functioning level and develop care plans and recommendations in conjunction with other professionals of the client’s legal team.  

  • Facilitating the completion of benefit entitlement applications, as necessary, to maximize the clients’ eligibility for benefits like Medicaid, and other potential benefits, attending appointments as requested by client, and working closely with the attorney on any appeals.  

  • Attend appointments with the attorneys and interact with clients and referral sources serving as witness and/or notary during signing appointments.  

  • Research community resources to ensure all firm information of facilities, resources, and materials are up to date to assist clients engaged in life care planning.   

  • Work to make client satisfaction a top priority, finding quick resolution to client complaints and concerns.  

  • Perform general operational tasks including but not limited to scheduling, answering the phone and forwarding messages, copying, scanning, mailings and postage, assisting drop-in clients, filing paperwork, handling voice mail messages, etc., all according to Firm policies and procedures.  

  • Represent the law firm to facilities and the health care community by visiting regularly, creating positive relationships with management and staff.  

  • Maintain confidentiality of all client and personnel information in accordance with O’Connell Law LLC policies. 

  • Alerting your Supervising Attorney to potential threats to the success of the firm. 

  • Perform any other duties assigned by supervisor. 

Qualifications 

  • Bachelor's degree in health care, related social work or human services, or another related field. 

  • 2-4 year's experience in geriatrics or another related field is preferred. 

  • Prior experience in care management, hospital or nursing home setting or related field preferred. 

  • Professional written, telephone and interpersonal skills required.
